Where we go
Lee Memorial, HealthPark, Gulf Coast Medical Center and Cape Coral Hospital — plus rehabilitation centres, nursing homes, skilled nursing, assisted living, memory care and hospice across Lee County. We also come to private homes where a family member is being cared for.
What usually gets signed: powers of attorney, healthcare surrogate designations, living wills and advance directives, deeds and property transfers, bank, insurance and retirement forms, guardianship and pre-need documents.

Valid photo ID
Unexpired, or two credible witnesses.
A signer can be frail, bedbound, on oxygen, hard of hearing, or unable to hold a pen steadily and still sign. We can work with all of that.
What we cannot do is notarize for someone who is unconscious, heavily sedated, or unable to communicate their own intent — even when the family agrees and the need is real. If we arrive and that is the situation, we will tell you honestly and there is no charge for the visit.
Witnesses
Many of these documents need two witnesses as well as the notary. Facility staff often cannot serve, and family members named in the document usually should not. Tell us when you book and we will sort it out before we arrive.
Social workers & case managers
If you place these requests regularly, call us directly. We answer our own phone, we know the buildings, and we can usually work around discharge timing.
